The importance of an updated medication history:

  • The US Department of Health and Human Services has declared that an electronic medication record is a method of reducing medication errors and is a "potential breakthrough." 

  • The disaster and chaos that Hurricane Katrina brought with it showed the need for an electronic, easily available patient medication record for healthcare providers. The Markle Foundation released its report (PDF format) on the Lessons Learned from the Katrina Health Disaster and outlined these recommendations (June 13, 2006).
